Grants Support Childhood Obesity Prevention, Diabetes Management for Native Hawaiians and Healthy Living/Active Living Program for Teens
Kaiser Permanente Hawaii today announced $140,000 in grant funding to three local community organizations/initiatives:  the “Hawaii 5-2-1-0” childhood obesity prevention initiative, Department of Native Hawaiian Health’s diabetes management program and YMCA’s “STRIVE” teen health program.
Grants include:
Hawaii 5-2-1-0 Initiative - $65,000 Grant
This initiative will promote healthier lifestyles in schools utilizing “Community Health Activators” (i.e. health educators, providers, teachers, etc.) empowered with tools known as “Go Kits” to enable healthy choices and active lifestyles in line with Dept of Education’s Wellness Guidelines. At least 10 schools on Oahu and the Big Island, reaching more than 2,000 children, will be targeted. Hawaii 5-2-1-0 is based at the University of Hawaii John A. Burns School of Medicine Department of Pediatrics and is led by HICORE, the Hawaii Initiative for Childhood Obesity Research and Education.
Department of Hawaiian Health and Kula No Na Poe Hawaii - $50,000 Grant

Kaiser Permanente’s grant will support DNHH and KNNPH’s community-based diabetes management and support for Native Hawaiians in the Hawaiian Homestead community.  Findings from this initial program will be shared for replication.
YMCA - $25,000 Grant
Grant will support YMCA’s STRIVE program for teens, integrating healthy eating and active living lessons using self-reflective processes, individual goal setting, community building, and interactive activities with peers and adult mentors.
